Amazon learned this in March

Amazon ran multiple high severity outages in a single week this March, including a six-hour failure that blocked checkout and pricing for millions of customers. The company traced one incident to an engineer acting on advice an AI agent had inferred from an outdated internal wiki.

The wiki was still sitting there. The agent read it. Nothing in between flagged that the page had stopped being true.

That is the mechanism in full. Retrieval ranks by relevance, and a retired pricing sheet is every bit as relevant to a pricing question as the current one.

Your agent has no way of knowing the world moved.

Storage economics made it worse. Longer context windows and persistent memory turned keeping everything into the cheapest option, so both versions of your pricing sit in the same store, waiting to be asked for.

Who owns what it keeps

Two in three enterprises are running agents on retrieval no one governs.
